Days of Our Lives spoilers for September 21, 2026 put Sarah Horton on the wrong end of the law, and the timing is almost rude. Six days ago she reported a drug company to a federal regulator. Monday she is the one facing legal problems. Nobody at that company has answered for a single death yet.
Here is what she actually did. On September 15 she sat in the Brady Pub with Brady Black and Xander Cook Kiriakis and told them she had filed an anonymous complaint with the FDA, in her own capacity as a doctor, alleging DiMera dumped an arsenic-tainted batch of Coriseal on Smith Island. Xander objected that nobody had consulted him. Sarah’s answer was that no one let her do anything, it was her decision.
The anonymous part lasted about ten minutes. The executive who runs DiMera’s pharmaceutical arm worked out that same episode where the complaint had come from, and warned Johnny DiMera it could put all of them in prison. By Friday the news had reached EJ DiMera, who heard it from his own assistant and walked straight out of the room. The wider week sits in our ongoing daily coverage of the show.

Days of Our Lives Spoilers: Xander Kiriakis and Brady Black Pick the Same Side
Monday is also the day Xander and Brady join forces, which is not a sentence this show gets to write often. One is her ex husband, the other is the man she lives with, and they have spent the year circling each other. The spoiler does not say what the alliance is for. It does not have to. Steve Johnson and Brady have been picking at the Coriseal case since the case that first cracked Salem open.
Reinforcements arrive Tuesday, and they arrive in a lab coat. Kayla Johnson steps in to defend Sarah, and by the end of the week the two of them are looking at whatever landed on her and deciding it does not add up. Circulating spoiler chatter is already using the word set-up.
Is EJ DiMera behind Sarah Horton’s legal trouble on Days of Our Lives?
Nothing published says so, and no character has said it out loud either. What is on the record is uglier and more useful. Back on July 17 EJ tried to have Sarah fired from the hospital, for the reason Xander said out loud at the time, that she was getting too close to the Coriseal truth. He failed. The person who stopped him, in that same stretch, was Kayla, who told him the board had just stripped his ownership rights out from under him.
So the doctor he tried to remove in July is the doctor in trouble in September, and the colleague who blocked him then is the one defending her now. My gut tells me that is not a coincidence the writers stumbled into. Here is the wrinkle, though. Friday showed us a man being ambushed by his own assistant with news he clearly had not seen coming, and the moment he left, she told a portrait of EJ’s late father that everything was playing out as planned. Reading between the lines, somebody in that building is a step ahead of EJ, not working for him.

Days of Our Lives Spoilers: Philip Kiriakis Comes Home and Theo Carver Unloads
Philip Kiriakis gets the cruellest beat of the day. He walks back into Salem off a win in Monte Carlo and the triumph curdles before the hour is out. Gabi Hernandez put up the money for that seat and sat at the table beside him, so whatever sours it, she wears it too. Don’t be surprised if the problem is who the money has to go to rather than whether he won it.

Elsewhere Alex Kiriakis spends the episode being the responsible one, which is new. Joy Wesley tries to duck another round of medical tests and Alex refuses to let her get away with it.
And then there is Theo Carver, who finally has somebody to talk to. He typed a resignation letter on September 16 and left it on a desk for Johnny to find, which is not the same as quitting, and as of Thursday nobody had read it. Monday he vents to Gabi. A man walking out on one company for breaking the law, talking to the woman who bankrolled a poker game to save a rival.
